**T& C for Referral Cash Voucher
- This e-voucher is valid only for first visit at Flower Moon TCM Clinics.
- Each person is only entitled to use one e-voucher regardless the number of visits.
- This e-voucher is not applicable to package holder.
- This e-voucher is only for consultation and treatments, not applicable for medication and products.
- This e-voucher must be fully utilized in a single transaction and any unconsumed value will be forfeited and is not refundable. E-voucher cannot be exchanged wholly or partly for cash.
- In the event that the value of the e-voucher used in a single transaction does not cover the full cost of the consultation or treatments purchased, the balance must be paid by the e-voucher holder.
- If all e-vouchers issued have been used up by families or friends, the equivalent amount of total value of e-vouchers will be credited into package holder’s package at ONE time ONLY.
- Flower Moon TCM Clinic reserves the right to vary these terms and conditions at any time without prior notice and at its sole discretion.
Follow-up Consultation $1 Benefit for Flower Moon Medical Package Holders (for package bought before 1 July 2025)
- This benefit is ONLY applicable for subsequent follow-up visits, which happens < ONE MONTH from the previous consultation.
- It does NOT apply to House-call Consultation.
- The above benefit is terminated if package value falls below treatment charges on that day.
- This benefit is with effect from 15 July 2023 until further notice.
- Flower Moon Clinic reserves the right to change these terms and conditions at any time without prior notice. In the event that any changes are made, the revised terms and conditions shall be posted on our website immediately.
