Procedure for House Call case
1. FMC counter staff to let patient fill in health declaration form (Google form) before house call.
2. FMC counter staff to provide patient’s ID ( eg C1234 from FMC), full name & house call address to staff who conduct house call.
3. FMC staff (who conduct house call) to record time in and out per MOH circular 2021-76_8jun21_ Clause 6.

4. FMC staff (physician who conduct house call) let patient scan QR code to fill in Health Questionnaires/ Treatment agreement.
The QR code will be provided by counter staff to FMC staff (physician who conduct house call) .

Protocols for management of ART COVID-19 antigen positive (Ag+) workflow &
public health actions required by employer.
If the employee is well but the test result is Ag+, the employee must self-isolate at home for the first 72 hours. Thereafter, take an ART again and end isolation if the result is negative. If the employee is unwell, he/she should see a doctor for further
evaluation. Employers are responsible to ensure appropriate management and adherence of Ag+ follow-up protocols is taken by employee.
